Support Staff

Job Summary
Assist clients with daily implementation of their Support Plans to maximize independent functioning with a safe and healthy home/community environment.  Assure client rights.

Key Services / Accountabilities

  • Assist clients in learning and performing objectives in the Support Plan.  Provide training and coaching to clients in areas such as personal interactions, personal care, social, leisure, community participation, and personal safety skills at home and in the community.  Recommend changes as required in the Support Plan.
  • Administer medications accurately and on schedule.  Ensure adherence to special dietary guidelines
  • Perform housekeeping and cleaning tasks to maintain a safe, healthy environment.
  • Serve as a member of a support team engaged in working effectively together to provide quality supports for clients in the most effective and efficient manner possible.  Effectively serve in team roles as assigned.  Communicate effectively and professionally with other team members.  Remain current with required training.
  • Maintain accurate and current written records.
  • Ensure activities and operations are carried out in compliance with CARF, DMH, OSHA, DOL, HIPAA and other regulatory and accreditation organizations.

 

Education & Experience Required

  • High school graduate.
  • Residential – None required.
  • Day Habilitation - One or more years working with MR/DD population, or completion of 24 hour DMH required training.

 

Physical Demands

  • Able to perform functions described in Key Services.
  • Employees assist persons served to stand from the ground, a sitting position, or out of a bathtub.  Employees are required to turn persons served in bed, reposition them in a wheelchair, or in other circumstances lift, or assist lifting, a person to maintain their safety and comfort. Will need to lift up to 50 pounds and assist with lifting, transferring or moving individuals up to 200 pounds with the assistance of adaptive equipment. This includes loading into and out of vehicles.
  • Able to drive an average vehicle, including a 12 passenger van. Current driver’s license and proof of insurance.
  • May be exposed to climatic conditions when transporting clients.

 

Mental Demands

  • Able to perform functions described in Key Services.
  • Able to appropriately and affectively assist individuals during crisis situations.
  • General Computer skills.

 

Language Skills

  • Proficient writing, speaking and communication skills in English.
  • Able to effectively present information and respond to questions from groups of managers, clients, and family/guardians.

 

Mathematical Skills

  • Able to perform routine math functions.
